Please read the contract in the window to your right and check the box below and enter your first and last name to agree to the terms of it. benefitRFP Agency Agreement I agree that by electronically signing this agreement that I am entering into a binding contract with benefitRFP for help on a case referenced above, and encompassing any peripheral* business that is generated by this relationship as it relates to this specific case. This case will be further identified and attached to this contract in the “CASE QUESTIONAIRE” . As levels of service sometimes change as a case progresses, I am agreeing in advance to any of the compensation splits hereafter described, and am bound by the description of service as it relates to the specific total compensation on the afore mentioned case. I further agree that this compensation will be paid directly to me and any compensation to other agents that worked on the case with me will be my responsibility and paid by me to them from my portion of the described compensation. benefitRFP Compensation Levels and Explanations ___ Referral Only: Agent receives 50% of total comp received by BenfitRFP Referral only is being referred into a case by an agent who is known by the client, and his referral opens the door for us to do our work. He needs to do no more than make the effective introduction. ___ Point of sales support: Agent receives 65% of total comp received by BenefitRFP Point of sale support is defined as a BenefitRFP representative having direct contact, in any form, with the client or one of the client’s representatives, board of directors, attorney, accountant, etc. The producer uses the internal BenefitRFP structures such as case design, our due diligence reporting systems, or implementation team. ___ Internal systems only sale: Agent receives 80% of total comp received by BenefitRFP Internal systems only sale is defined as the producer having all conversations with the client. BenefitRFP provides case design, due diligence reporting. Producer handles implementation and plan service. ___ Product only sale: Agent receives 95% of total comp received by BenefitRFP Producer tells us up to three carriers to run a basic financial model. No plan due diligence provided. No other internal systems are engaged. I agree to all provisions described in this agreement, including the Compensation Level initialed by me herein, and further agree that my electronic and/or faxed signature binds me to all its provisions as applied to the case described in the first paragraph and the attached “CASE QUESTIONAIRE”. *Peripheral business in this context means any business developed as a result of this agreement and the work done for the case described.
